New Client Welcome Center

We are pleased in your confidence in selecting us a your provider.  It is our goal to make your experience in receiving our services efficient, personal and helpful.  Unfortunately, the process of filling out all the necessary forms is tedious, it is important in providing needed information for your care, assuring that insurances cover your expenses and that we honor your privacy.

Registration/Intake Forms

• Client Registration Form (PDF) - All new clients/patients need to fill out this form.
• Treatment Consent and HIPPA Form (PDF) - All new client/patient also need to fill out this form which indicates that you are aware of the HIPAA privacy regulations, have agreed to treatment and given us a way to communicate with you. 
• Adult Biopsychosocial History Form (PDF) - New patients need to complete this form so your provider will know more about your family, medical, and social histories. 
• Child/Adolescent Biopsychosocial History Form (PDF) - like the adult form, we need this one to be completed by the patient's parent or legal guardian.
• Physician Coordination of Care Form (PDF) - If you are seeing a therapist you need to allow or deny permission to share information with your primary care physician.
